Dear Valued FishKiss Fish Spa Guest, FishKiss, LLC takes pride in providing our guests with the best experience and customer service possible. The safety and sanitation of our guests and our fish are extremely important to us. In order to provide a safe and sanitary experience, here are the safety protocols used to protect each guest: Safety for our Valued Guests Washing of each guest’s feet and/or hands before each FishKiss Experience (fish pedicure). Washing and sanitizing of each guest’s feet and/or hands with warm water and antibacterial soap after each FishKiss Experience (fish pedicure).   Each guest will receive spa flip flops, never reused, and never worn twice. An herbal towel wrap and lotion will be used.Safety/Sanitation for our Fish and Tanks At least 25% of each aquarium water is recycled weekly. Double aquarium filtration system with carbon bags inside each filter to clean  aquarium water and keep fish healthy. Ammonia levels are monitored each morning and kept below .5 PPM for spa use. Water pH levels are checked daily and are kept between 7.0-7.6. The water is heated constantly to 72-80 degree Fahrenheit to keep the fish healthy. Each tank will have two air stones with a minimum of 1.5 liters of air. No open wounds, scratches, or lacerations will be allowed to be submerged in any of the fish aquariums.  By signing below, you expressly agree to the following: I agree to a photo and video release of my likeness for presentation under any legal condition, including, but not limited to publicity, copyright purposes, illistration, advertising, and web content. I understand that there shall be no payment for this release. I understand that no royalty fee, or other compensation shall be payable to me by reason of such use. I understand that with my authorization below the photographs and videos may never ben revoked. I agree to receive SMS text message and email updates and promotions (I understand I can later opt-out of these promotional email and SMS offers if I so choose), and I declare that I do not have any open wounds, scratches, or lacerations of any kind on the body part that will be submerged in the FishKiss tank. Furthermore, I understand that if I use FishKiss Fish Spa’s services I will come in contact with Garra rufa. As a fish, Garra rufa may carry bacteria or other diseases. Additionally, interacting with other individuals in a spa-like setting may carry additional risks of infection, disease, or illness. Regardless of any precautions taken by FishKiss Fish Spa, I understand that there may be a risk of undesirable reactions to the services which may occur, including infections, rashes, irritation of the skin, and allergic reactions, etc. Furthermore, I allow employees at FishKiss Fish Spa to come in contact with my bare hands and/or feet with their hands for cleaning and sanitizing purposes during the FishKiss Experience (fish pedicure) and all other services provided at FishKiss Fish Spa. Accordingly, I acknowledge that:  I am voluntarily participating in the services with a full knowledge and understanding of the potential risks involved in the services. I do not have any cuts, bruises, or infections that may come in contact with the Garra rufa or the water in the treatment tank. FishKiss Fish Spa’s employees may examine my chosen treatment area for any cuts, bruises, or infections before beginning the services and at any time during the services. I am not suffering from any condition, impairment, disability, disease, infirmity, or illness that should prevent me from using the services. I voluntarily assume all responsibility and liability for using the facilities and equipment and participating in the services offered at Fish Kiss Fish Spa’s locations. In consideration of being allowed to participate in Fish Kiss Fish Spa’s services, I personally assume all risks involved in or resulting from using the services or Fish Kiss Fish Spa’s facilities. I waive and release, now and forever, all claims and causes of action against Fish Kiss Fish Spa, its officers, agents, volunteers, employees, representatives, consultants, executors, and all others directly or indirectly connected with Fish Kiss Fish Spa, from any and all injuries sustained from the services, including any medical condition, aggravation of a pre-existing medical condition, or infection, and any and all other damages or injuries which I sustain in any way from the direct or indirect result of using the services. I understand that by signing this document, I only have to sign this document once, and all other times I choose to return back to FishKiss Fish Spa, I am held under the same covenant made each consecutive time. I further hold FishKiss Fish Spa harmless from any loss to personal property that is lost or stolen while I am present at Fish Kiss Fish Spa’s locations.
